for Registration
- Current proof of
income
- Paid weekly,
last four consecutive pay stubs
- Paid biweekly
or twice a month, last two consecutive pay stubs
- Paid monthly, last pay stub
- A letter from
your employer on company letterhead stating your weekly hours,
rate of pay and schedule of pay (if not paid by check)
- Self-employed,
a copy of your most recent income tax return
- Documentation
of school or training program, including hours, if applicable
- Documentation
of state assistance, if applicable
- Birth Certificates
for all children and underage parents living in the household (full size)
- Two proofs of address:
if you own your home, a current mortgage statement and utility bill. If you rent your home, a current lease and utiltity bill
- $35.00 non-refundable
enrollment fee. Exact cash, Master Card or Visa will be accepted
(NOT APPLICABLE FOR HEAD START PROGRAM)
- A completed physical
form for your child that is current within one (1) year
- Childs medical
insurance card
- At least two local emergency
contact names and daytime phone numbers
- The Social Service
Office has the right to request additional information or documentation
as needed in order to complete registration.
Dental exam
- Child support (if applicable).
- IEP/IFSP documentation (if applicable)
- Social Security income (if applicable)
